When You’re Not Sure What to Do Next, We’ll Help You Find Your Way.
You’re trying to do the right thing for someone you love, but it’s hard to know what the next step should be and when to take it.
Hospice can feel like a big step, especially when you’re already stretched thin, physically and emotionally. Many families wait to call, not because they want to, but because they’re unsure what hospice really offers or how it can help. But in all our years of caring for families, we’ve never heard someone say they called us too soon.
At Blue Bonnet, we meet you with compassion and clarity. We walk beside you from day one, helping you understand your options, being there for your loved one with compassion, and giving your whole family the support you deserve.
No pressure, just honest answers from an experienced nurse
Why Families Trust Blue Bonnet
Families don’t choose us for what we do, they choose us for how we do it. Here’s what makes Blue Bonnet different:
-
We’re not a distant corporation or chain; we’re your neighbors.
-
Families tell us it’s a relief to see the same nurses visit again and again. Our team is consistent, personal, and committed, no revolving door of staff.
-
From the very first call, you’ll talk directly with our owner or a member of our nursing staff; no phone trees, no runaround.
-
We’ve earned the trust of local families, physicians, and care partners by doing things the right way, with warmth, clarity, and compassion.
-
For assisted living teams, physicians, and caregivers, we fit into your rhythm, communicate clearly, and follow through.
“Blue Bonnet helped our family so much. We were expecting our Mom to live in assisted living and she declined rapidly after a few weeks. We did not know what to do for her and Blue Bonnet came in and took over. Finally, we felt like we were doing the best thing for her. They handled her care for 8 months and we never had a complaint!”
“The care provided to my father improved his qualify of life and length of life. Hospice was also of tremendous benefit to me as the primary caregiver”
How we help
Getting the care your loved one deserves is easier than you think.
Step #1.
Call and Schedule a Family Meeting.
First and foremost, we listen. A member of our staff is available 24/7, ensuring immediate assistance without the interference of an answering service. We want to hear from you and pair you with the right resources, whether that includes our services or not.
Step #2.
Family Meeting.
We will meet with you in person or over the phone to discuss your family’s needs. Then, we will assess and craft a personalized care plan. Patient wishes are paramount, and we honor them with unwavering respect. Limiting our census ensures that each patient receives the personal attention they deserve.
Step #3
Begin Care.
Based on the personalized care plan, care can start right away. Our dedicated team includes:
Supervision by a Medical Director
Director of Nursing with 30+ years of hospice experience that you will have direct access to.
Registered Nurse Care Managers.
Guidance and emotional support from our Social Workers.
Spiritual Counseling and support from our Chaplains.
Certified nurse aides providing attentive hygiene care
Respite care to provide relief for caregivers.
Access to essential Medical Equipment, supplies, and medications.
-
We evaluate every situation and customize a care plan to meet the needs of the patient and family. By limiting our patient census, we ensure each patient receives the personal attention they deserve.
An example care plan would include, but not limited to:
Two nursing visits per week.
Three nurses aide visits per week for hygiene care.
24/7 assistance provided by someone on OUR team. We do not outsouce after hours calls to a answering service.
Monthly visits from our Social Worker.
Weekly, Monthly or as needed visits from our Chaplain.
Continuous pain and symptom management
Medication changes and management
Required Medical Equipment delivered and set up.
-
We meet you where you’re at. Most commonly work with patients at home or in an assisted living facility. If you or your physician feels that it’s the right time to evaluate hospice options, give us a call and we can talk you through our process.
Our home base is in Wimberley, Texas, but provide hospice services to all of the surrounding cities including our Houston Location.

Hospice care is provided at no cost to our patients. For individuals aged 65 and older with Medicare benefits, hospice services are fully covered. At Blue Bonnet, we also accept Medicaid and most major commercial insurance plans, ensuring accessible care for all who need it.
Over the past 10+ years, we have provided over $300,000 worth of care to people who do not have a way of funding care.
-
If you are being discharged from the hospital with a hospice order or are eligible for hospice care, we encourage you to contact us. Our team can coordinate with your hospital case manager to arrange transportation home, ensure medical equipment is delivered, and have a nurse ready to assess your needs upon arriving home.
We can also send a nurse, social worker, or another member of our team to the hospital to discuss your care options in person.
-
When a patient is on Hopsice with us, we will provide essential medical equipment and supplies. During our “family meeting” we will access what items you need and arrange to have them delivered promptly.
-
Respite Care allows for us to provide extra care to a patient to give relief to a caregiver. Whether you are a caregiver that needs extra assistance, need time to run errands or need to leave town for a few days, we can arrange extra help!
We understand the critical service caregivers provide their loved ones, and we are here to help lighten the load and help in any way that we can.
