What is Hospice care?

Hospice care is a specialized form of care focused on comfort, support, and quality of life for individuals facing a life-limiting illness. It is not about giving up—it’s about living fully, free from pain and discomfort, surrounded by compassionate support. Hospice addresses physical, emotional, and spiritual needs while providing families guidance and relief. Our goal is to create a peaceful and meaningful journey for everyone involved.

Can hospice care be provided at home?

Absolutely. We specialize in bringing care to wherever your loved one feels most comfortable, whether at home, in an assisted living facility, or in another setting. We’ll create a care plan tailored to their needs and preferences.

What is the process of setting up Hospice care?

We understand that time is of the essence when your loved one is in need. Our team is available 24/7 to answer your call and coordinate care. In many cases, we can begin services on the same day. Call us, and we will start mapping out a plan for your loved one.

Care will typically begin with a visit from one of our wonderful nurses. Based on the initial assessment and after listening to the needs of the family/patient, we will also have essential medical equipment, supplies, and medications delivered.

What if we’re not sure if it’s time for Hospice?

Deciding on hospice can feel overwhelming. If you’re uncertain, we can provide a free consultation to discuss your loved one’s condition and answer your questions. We’ll help you make the best decision for your family without pressure or judgment. We also have extensive experience and resources in all things caregiving. Whether or not we are the solution, we will ensure we point you in the right direction.

How is Hospice paid for?

Hospice care is provided at no cost to our patients. For individuals aged 65 and older with Medicare benefits, hospice services are fully covered. At Blue Bonnet, we also accept Medicaid and most major commercial insurance plans, ensuring accessible care for all who need it.

Over the past 10+ years, we have provided over $300,000 worth of care to people who do not have a way of funding care.

What does a typical Hospice care plan look like?

We evaluate every situation and customize a care plan to meet the needs of the patient and family. Limiting our patient census ensures each patient receives the personal attention they deserve.

An example care plan would include, but not limited to:

  • Two nursing visits per week.

  • Three nurses aide visits per week for hygiene care.

  • 24/7 assistance is provided by someone on OUR team. We do not outsource after-hours calls to an answering service.

  • Monthly visits from our Social Worker.

  • Weekly, Monthly, or as-needed visits from our Chaplain.

  • Continuous pain and symptom management

  • Medication changes and management

  • Required Medical Equipment delivered and set up.

What if I don’t have the medical equipment that I need?

When a patient is on Hospice with us, we will provide essential medical equipment and supplies. During our “family meeting,” we will assess what items the patient needs and arrange to deliver them promptly.

What if I am being discharged from the hospital?

If you are being discharged from the hospital with a hospice order or are eligible for hospice care, we encourage you to contact us. Our team can coordinate with your hospital case manager to arrange transportation home, ensure medical equipment is delivered, and have a nurse ready to assess your needs upon arriving home.

What is Respite Care?

Respite Care allows us to provide extra care to a patient to give relief to a caregiver. Whether you are a caregiver who needs additional assistance, needs time to run errands, or needs to leave town for a few days, we can arrange extra help!

We understand the critical service caregivers provide their loved ones, and we are here to help lighten the load and help in any way we can.

What happens if my loved one’s condition improves?

Hospice care is not about giving up hope—it’s about quality of life. If your loved one’s condition stabilizes or improves, they can transition out of hospice care. Our team will work with you to adjust their care plan as needed.

Where can you provide care?

We meet you where you’re at. Most commonly work with patients at home or in an assisted living facility. If you or your physician feels that it’s the right time to evaluate hospice options, give us a call and we can talk you through our process.

Our home base is in Wimberley, Texas, but we provide hospice services to all surrounding cities, including our Houston Location.
